This card is meant to be used by healthcare professionals (ex: MDs, DOs, ARNPs, PAs) to guide the decision making process when determining whether or not to consult Occupational Therapy (OT) and/or Physical Therapy (PT) services for a patient. Ancillary healthcare professionals who engage in interdisciplinary dialogue regarding the care of patients (ex: RNs, SLPs, PTs/PTAs, OTs/COTAs, RTs) may find this card helpful when determining when or if an Occupational Therapy or Physical Therapy consult should be suggested to the patient’s overseeing physician or medical team.
This reference is most applicable for adult (18yrs+) patient populations and is relevant to both inpatient (ex: hospital) and outpatient settings.
(Note: Some US states allow patients to attain direct access to Occupational Therapy or Physical Therapy services. In US states where direct access is illegal a physician or medical team extender must first refer the patient or place a consult for Occupational Therapy and/or Physical Therapy services, depending on the specific practice setting and/or facility.
